When booking an appointment, the following data is collected:

- • Phone number
- • Date of birth (optional)
- • Gender (optional)
- • Type of consultation (in-person / online)
- • Notes or reason for consultation (optional)
- • How you heard about the practice (optional, closed list)
The field "How did you hear about Paulo?" is optional, offers a closed list of answers and accepts no free text. It records neither the name nor the profession of any referrer. If Paulo separately establishes that you had visited the website before booking, he may record that in your booking. This information is used solely for aggregate measurement and is never used for any diagnosis or clinical decision. No health data is collected via the online form. Medical and therapeutic information is handled exclusively within the therapeutic relationship, subject to professional secrecy.
